Planning can help older adults live healthy at home

BEDFORD, MA, November 11, 2020 /Neptune100/ — Hospital admissions at any time are unwelcome. During a pandemic, becoming an inpatient for reasons unrelated to COVID-19 is even more concerning while virus infection rates resurge and flu season is approaching. A new handbook, by a Bedford-based home health care service provider, outlines ways to reduce the chances of hospital stays for older adults, by following simple precautions.

“Thriving at Home – A Handbook for Preventing Hospital Stays,” written by Dave Tasto, Certified Dementia Practitioner® and owner of Assisting Hands® Home Care, includes easy-to-follow advice for older adults to keep their home environments safe and manage their chronic illnesses, both of which are proven ways to reduce incidents of hospital stays. The title also refers to a movement in home care which supports people living independently for as long as safely possible through the availability of a continuum of services.

“Being proactive is a critical element for older adults to maintain health and happiness at home,” says Tasto. “They include people with chronic illnesses, who if they follow plans designed to manage their diabetes, heart disease and chronic obstructive pulmonary disease (COPD), for example, can reduce incidents of hospitalization. Older adults tend to encounter many possible health scenarios and this handbook can help them to make the right decisions and see healthy, long-term outcomes for living well at home.”

Moreover, Tasto pointed out, one in five Medicare patients are readmitted to the hospital within 30 days. “Readmissions place unnecessary stress on patients, their caregivers, and the health care system. We know that much of this can be prevented.” The handbook also addresses how to prepare for a hospital stay in the COVID-19 era and more.

Thriving at Home provides checklists to help people prepare for a necessary hospital stay, adjust to recovering successfully following discharge from the hospital, and how to minimize the risk of readmission.
